Lodge Expenditure Form
Use this form to submit a request for being refunded on a lodge expense that you paid, in which the lodge has approved to re-pay you. This file is in pdf form.

Master Event Permission Form
Use this form to authorize you/your son to attend events with Lakota Lodge and permit transportation. This is required activities in which the activity manager mandates. This file is in doc form (word document).

Class One Physical Form
This form informs the Lodge of special medical conditions a scout/scouter may have, in case of an emergency. This file is in pdf form and has been released by National Boy Socuts of America.
